There's a very specific kind of anxiety that can show up the moment you start thinking about having kids.

Suddenly there are supplement stacks, elaborate preparation routines, people six months out from conceiving already optimizing everything, and a completely normal moment of "maybe I should start learning about this" like you're already behind.

We want to offer a different version of this conversation.

"Trimester Zero" is the name that's been given to the pre-conception window. This is roughly the three to six months before you try to conceive. 

The underlying idea is genuinely good: this period matters. It’s worth paying attention to, and it tends to fall completely through the cracks of normal healthcare. Most of us get no guidance on it whatsoever, so the instinct to take it seriously is minimal.

The content you're seeing around it, though, has gotten a lot bigger than the evidence.

What does preconception health actually require?

The honest answer is that the list is shorter than the internet would have you think.

According to the CDC, starting folic acid (or folate) at least one month before conception significantly reduces the risk of neural tube defects. This is a serious birth defect that affects the brain and spine that develop very early in pregnancy, often before most of us even know we're pregnant.

Beyond that: be mindful of alcohol, don't smoke, and if you're managing a chronic condition, loop in your doctor about what's worth monitoring going into this period.

That's what has solid, replicated research behind it for most of us.

Everything else you're seeing, from the elaborate supplement protocols, the red-light therapy, the extensive cycle optimization routines, range from potentially useful to simply unproven for most of us. Some of it may genuinely help depending on your specific health context, but none of it is required the way it gets presented.

Does it look the same for everyone?

Not even close! And this is the part that almost never makes it into the content.

If you're managing an autoimmune condition or PMOS (Polyendocrine Metabolic Ovarian Syndrome, formally known as PCOS), your preconception conversation looks different. If you've been on hormonal birth control for years and want to understand your natural cycle, you're in a different place than someone who's been tracking theirs for ages. If you have easy access to comprehensive bloodwork, your options look different than if you don't.

The preparation that makes sense for you depends on your health history, your access, and where you actually are right now. If you do a thorough baseline panel, that's a valid version. So is starting folate, cutting back on alcohol, and leaving it there.

What about your partner?

Trimester Zero content is almost entirely directed at those of us with uteruses, which means an important part of the picture tends to get left out.

Sperm takes approximately three months to develop, which means the lifestyle choices a partner makes in the lead-up to conception actually matter but not in a complicated way. Smoking, heavy alcohol use, excessive heat exposure, and nutritional gaps all have documented effects on sperm quality. A few straightforward adjustments during this window – and, for those who want a baseline, a semen analysis – is a reasonable parallel to what we'd suggest on their end.

If you have a partner in this, Trimester Zero should be a shared experience.

So where do you actually start?

Folate. That's the clearest, best-evidenced first step. From there, a short conversation with your GP to flag anything specific to your health history. And, if you want to go a bit further, a baseline blood panel covering things like thyroid, iron, and vitamin D gives you a real picture of your starting point.
